Please contact your insurance company to determine if you need an authorization or referral prior to surgery. If not obtained, you may be responsible for the payment of the surgery. We do our best to contact your insurance company prior to your appointment but this is not always possible.

Most insurance policies cover the cost of Mohs surgery, however you should check with your carrier for exact information related to their schedule of payments. On your first visit, please bring a copy of your I.D. card, Group Number, Policy name, and correct mailing address of your insurance carrier. We accept assignment on Medicare. Patients with insurance carriers other than Medicare will be billed, if your insurance plan has a deductible you will asked to pay a portion of this at the time of service. We accept cash, checks, Visa, and Master Card.
