Upon scheduling an appointment, you will be sent a packet with additional information regarding Mohs Surgery, your type of cancer, day of surgery instructions, and directions to our center. You may also print your registration forms and packet information from the links on this page. At the time of scheduling, the Seattle Skin Cancer Center will review the information with you and will be readily available to answer any further questions and address concerns.
If you are unable to keep the scheduled surgery, please contact the office and be sure to specify that you need to reschedule your surgery appointment.
In consideration of other patients during this cold and flu season, we would ask that you, and anyone attending the appointment with you, be free of cough or cold symptoms.
